  [摘要] 目的 探讨脑电图(EEG)、脑干听觉诱发电位(BAEP)、视觉诱发电位(VEP)在小儿脑性瘫痪早期诊断中各自的作用及其相互关系。 方法 对75例小儿早期脑性瘫痪的EEG、BAEP、VEP检测结果进行回顾性分析。 结果 EEG异常率为73.33%,BAEP异常率为81.33%,VEP异常率为30.67%,EEG异常率和BAEP异常率与VEP异常率比较,差异有高度统计学意义(P < 0.01),EEG异常率与BAEP异常率比较,差异无统计学意义(P > 0.05);EEG异常率与小儿脑性瘫痪(CP)类型有关;BAEP异常率与高危因素有关。 结论 脑性瘫痪早期应用EEG、BAEP、VEP检查,对脑性瘫痪早期诊断有一定的参考价值,他们之间能起到互补的作用, 对临床综合分析更有意义。

  Comparative analysis of electroencephalogram, brainstem auditory evoked potentials and visual evoked potential in early diagnosis of children cerebral palsy

  [Abstract] Objective To research the function and correlations of electroencephalogram (EEG), brainstem auditory evoked potentials (BAEP) and visual evoked potential (VEP) in early diagnosis of cerebral palsy of children. Methods The EEG, BAEP and VEP test results of 75 children who were diagnosed as early cerebral palsy were analyzed retrospectively. Results Abnormal rate of EEG was 73.33%, abnormal rate of BAEP was 81.33%, abnormal rate of VEP was 30.67%, the comparing of abnormal rate of EEG and BAEP with VEP had statistical significance (P < 0.01), the comparing of abnormal rate of EEG with BAEP was not statistical significant (P > 0.05); abnormal rate of EEG was related to the type of CP, BAEP abnormal was related to the rate and high risk factors. Conclusion The using of EEG, BAEP, VEP show reference value at the early diagnosis of cerebral palsy, they play the role of complementary function and good for clinical comprehensive analysis.

  小儿脑性瘫痪(cerebral palsy,CP)是指自受孕开始至婴儿期非进行性脑损伤和发育缺陷所导致的综合征,主要表现为运动障碍和姿势异常[1],常伴有智力低下、癫痫、语言障碍、视听障碍等损害,是小儿神经系统常见的一种疾病。本病是导致儿童残疾的最常见、也是极为严重的疾病之一,不但增加家属经济和心理负担,而且也严重影响家属的工作和生活质量[2]。CP早期诊断,早期干预,对提高本病的康复率、最大限度减少致残率至关重要。本文对2009年 1月~2011年12月在我院行脑电图(EEG)、脑干诱发电位(BAEP)、视觉诱发电位(VEP)检测的早期CP患儿75例,对其检测结果进行回顾性分析,现报道如下:
  1 资料与方法
  1.1 一般资料
  选取2009年 1月~2011年12月符合小儿CP诊断标准[1]患儿75 例,其中,男41 例,女 34例,年龄1~6个月,平均 2.2个月。存在高危因素:新生儿窒息28例,核黄疸22例, 颅内出血13例,早产难产儿9例,新生儿期感染惊厥6例,癫痫9例,小头畸形1例,原因不明10例。临床类型:痉挛型37例(49.33%),肌张力低下型 18 例(24.00%),手足徐动型11例(14.57%),混合型 6例(8.00%),未能分类型3例(4.00%)。并发症:听力障碍26例,视力障碍4例,癫痫6例,有些病例有2种以上并发症。
