手法调整膝关节结构微絮乱治疗单侧膝关节滑膜炎的疗效观察

　　[摘要]目的探手法调整膝关节结构微絮乱治疗单侧膝关节滑膜炎的疗效。方法选取2019年6月至2021年6月于广州市中西医结合医院住院诊断为单侧膝关节滑膜炎的60例患者作为研究对象。将所有患者根据入院顺序编号，编为1～60号，随机分为观察组与对照组，每组各30例。两组患者均接受常规健康宣教以及康复训练。在此基础上，对照组患者接受塞来昔布治疗，观察组患者接受手法调整膝关节结构干预。记录治疗前后两组患者双膝髌骨上缘高度差、治疗后疼痛情况、肿胀程度改善情况以及住院情况。结果治疗前，两组患者双膝髌骨上缘高度差差异无统计学意义（P>0.05）。治疗后，两组患者双膝髌骨上缘高度差均较治疗前有所下降，差异有统计学意义（P<0.05）。进一步组间比较显示，治疗后观察组患者双膝髌骨上缘高度差小于对照组患者，差异有统计学意义（P<0.05）。治疗后，两组患者VAS评分分布差异有统计学意义（P<0.05）。观察组患者患肢关节肿胀程度改善有效比例高于对照组，差异有统计学意义（P<0.05）。观察组患者住院时间和住院费用均少于对照组患者，差异有统计学意义（P<0.05）。结论手法调整膝关节结构微絮乱治疗单侧膝关节滑膜炎能够有效减少患者双侧膝髌骨上缘高度差。此外，还能有效减轻患侧膝关节的肿胀程度与疼痛，与寒来昔布治疗相比，具有效果确切，术后恢复快，治疗费用少的优势，值得推广应用。

　　[Abstract]Objective To explore the efficacy of manipulation adjustment in micro-disorders of knee joint structure on unilateral knee joint synovitis. Methods A total of 60 patients with unilateral knee joint synovitis hospitalized in Guangzhou Hospital of Integrated Traditional Chinese and Western Medicine from June 2019 to June 2021 were selected as the subjects. All patients were numbered from 1 to 60 according to the order of admission and randomly divided into the observation group and the control group，with 30 cases in each group. Both groups received routine health education and rehabilitation training. On this basis，patients in the control group received celecoxib，while patients in the observation group received the intervention of manipulation adjustment of knee joint structure. The height difference of the upper edge of the patella between both knees before and after treatment，the pain，the improvement of swelling degree and the hospitality after treatment in the two groups were recorded. Results Before treatment，there was no significant difference in the height difference of the upper edge of the patella between both knees in the two groups （P>0.05），which was comparable. The height difference of the upper edge of the patella between both knees in the two groups after treatment was lower than that before treatment，with statistically significant differences （P<0.05）. The further inter-group comparison showed that after treatment，the height difference of the upper edge of knees and patella was lower in the observation group than that in the control group，with statistically significant differences （P<0.05）. After treatment，there were statistically significant differences in the distribution of VAS scores between the two groups （P<0.05）. The effective rate of the improvement of swelling degree in affected joints was higher in the observation group than that in the control group，with statistically significant differences （P<0.05）. The hospitalization time and hospitalization expense were lower in the observation group than those in the control group，with statistically significant differences （P<0.05）. Conclusion The manipulation adjustment of micro-disorder of knee joint structure can effectively reduce the height difference of the upper edges of the patella between both knees in patients with unilateral knee joint synovitis. In addition，it can effectively relieve the swelling and pain of the affected knee joint. Compared with celecoxib，it has the advantages of definite effects，quick postoperative recovery and low treatment cost，which is worthy of popularization and application.
