南宁市0~3岁牛奶蛋白过敏症患儿生长发育研究及全程营养管理疗效观察

  [摘要] 目的探全程营养管理对儿童牛奶蛋白过敏症(CMPA)的疗效。方法选取2019年 3月至2020年 6 月在南宁市第八人民医院等单位治疗的750例 CMPA 患儿为研究对象,随机分为研究1 组和研究2 组,每组各375例,选取同时期健康体检的非 CMPA 儿童600例为对照组。对照组正常喂养,研究2 组按常规方法进行营养管理,研究1 组由儿科营养医生全程指导,个体化补充维生素 A、维生素 D 及锌等营养,观察三组生长发育情况及营养指标差异。结果干预前 CMPA 患儿身高、体重、维生素 A、维生素 D、锌、血清铁、视黄醇结合蛋白及 IgG 水平低于非 CMPA 儿童,而生长发育迟缓及营养不良发生率高于非 CMPA 儿童,差异有统计学意义( P <0.05)。干预后,研究1 组 CMPA 患儿与健康儿童的身高、体重、维生素 A、维生素 D、锌、血清铁、视黄醇结合蛋白及 IgG 水平一致高于研究2 组,血IgE、嗜酸细胞数低于研究2 组,差异有统计学意义(P <0.05)。结论南宁市0 ~3 岁 CMPA 患儿普遍存在营养不良、生长发育迟缓及维生素 A、维生素 D 及锌缺乏现象,专业儿保医师全程营养管理可促进患儿生长发育。

  Study on the growth and development of 0-3-year-old children with cow’s milk protein allergy and observation on the effect of whole-course nutrition management in Nanning

  [Abstract] Objective To explore the effect of whole-course nutrition management on cow’s milk protein allergy (CMPA) in children. Methods A total of 750 children with CMPA in the Eighth People’s Hospital of Nanning and other institutions from March 2019 to June 2020 were selected as the research subjects. They were randomly divided into study group 1 and study group 2, with 375 cases in each group.600 non-CMPA children who underwent physical examination in the same period were taken as the control group. The control group was fed normally, the study group 2 received nutrition management according to conventional methods, and the study group 1 received the whole-course guidance of pediatric nutritionists and individualized supplement of vitamins A, D and zinc. The growth and development and nutritional indexes in the three groups were observed. Results Before the intervention, the height, body weight, levels of vitamins A, D, zinc, serum iron, retinol-binding protein and IgG in CMPA children were lower than those in non-CMPA children, while the incidence of growth retardation and malnutrition was higher than that in non-CMPA children, with statistically significant differences (P <0.05). After the intervention, the height, body weight, levels of vitamins A, D, zinc, serum iron, retinol-binding protein and IgG of CMPA children in the study group 1 were consistent with those of healthy children, but higher than those in the study group 2, and the levels of blood IgE and eosinophil counts were lower than those in the study group 2, with statistically significant differences (P <0.05). Conclusion Malnutrition, growth retardation and vitamins A, D and zinc deficiencies are common in 0-3-year-old children with CMPA in Nanning. The whole-course nutrition management by professional child-care physicians can promote the growth and development of affected children.
