透明帽辅助结肠镜对腺瘤检出率影响的临床观察

　　[摘要]目的探透明帽辅助结肠镜对腺瘤检出率的影响。方法选取2020年9―12月在北京市石景山医院进行结肠镜检查的480例患者作为研究对象，根据随机信封抽签原则将患者分为透明帽组与对照组，每组各240例。透明帽组采取透明帽辅助结肠镜检查，对照组采取传统结肠镜检查。比较两组结直肠腺瘤检出率，记录腺瘤的位置与形态;比较两组的进镜时间、退镜时间及患者的满意度。结果透明帽组的结直肠腺瘤检出率为32.50%，高于对照组的16.25%（ P <0.05）。两组检出结直肠腺瘤的部位比较差异有统计学意义（ P <0.05），形态比较差异无统计学意义（ P >0.05）。透明帽组的进镜时间和退镜时间均短于对照组（ P <0.05）。透明帽组结肠镜检查的满意度为99.58%，高于对照组的92.50%（ P <0.05）。结论透明帽辅助结肠镜能提高结直肠腺瘤检出率，缩短进镜时间和退镜时间，提高患者满意度，可为提升结肠镜检查质量提供新的思路。

　　Clinical observation on the effect of detection rate of adenoma by transparent cap-assisted colonoscopy

　　[Abstract] Objective To investigate the impact of transparent cap-assisted colonoscopy on the detection rate of adenoma. Methods A total of 480 patients who underwent colonoscopy admitted to Beijing Shijingshan Hospital from September to December 2020 were selected as the research subjects. According to the principle of random envelope drawing， patients were divided into the transparent cap group （n=240） and the control group （n=240）. The transparent cap group was examined with transparent cap-assisted colonoscopy， while the control group was examined with conventional colonoscopy. The detection rate of colorectal adenoma between the two groups was compared， and the location and shape of the adenoma were recorded; the average entry time of colonoscopy， the withdrawal time of colonoscopy and the patient's satisfaction were compared between the two groups. Results The detection rate of colorectal adenoma in the transparent cap group was 32.50%， higher than 16.25% in the control group （P <0.05）. There was a statistically significant contrast difference in the location of colorectal adenoma between the two groups （P <0.05）， and there was no statistically significant contrast difference in the morphology （P >0.05）. The average entry time and withdrawal time of colonoscopy in the transparent cap group were both less than those in the control group （P <0.05）. The satisfaction with colonoscopy in the transparent cap group was 99.58%， higher than 92.50% in the control group （P <0.05）. Conclusion Transparent cap-assisted colonoscopy can improve the detection rate of colorectal adenoma， shorten the entry time and withdrawal time of colonoscopy， and improve patients'satisfaction， which can provide new ideas for improving the quality of colonoscopy.
